Reimbursement of healthcare costs: how does it work in Belgium?

In Belgium, the mandatory healthcare insurance system enables those who are insured to access high-quality healthcare. The insurance covers the cost of numerous medical services including doctors’ appointments and medication. The Expat Welcome Desk explains how you can join the healthcare system and how much you can expect to be reimbursed.

Register with a mutual insurance scheme

In Belgium, membership of the healthcare insurance system is not automatic; you are required to register with a “mutual” health insurance organisation (mutualité in French, ziekenfonds in Dutch) and pay a subscription of +/- €10/month.

The principal role of these mutual organisations is to provide a partial reimbursement of medical and health costs, and to provide income replacement in the event of incapacity for work or disability (sickness, maternity leave, etc.). This is called compulsory insurance. It is regulated by law and is identical for all mutual organisations.

Mutual organisations also provide supplementary insurance, which offers various advantages and services in addition to those offered by compulsory insurance. These include fixed-rate reimbursements for acupuncture, denture costs, eye surgery and frames for your glasses. This complementary top-up insurance varies from one mutual organisation to another. It’s up to you to compare and decide which insurer is best for you!

Mutual organisations are not-for-profit associations grouped into national unions:

You can also choose to enrol with a regional office of CAAMI (Caisse Auxiliaire d’Assurance Maladie Invalidité). This organisation is a public institution that fulfils the same role as the mutual associations and provides similar services, except for supplementary insurance.

PLEASE NOTE that since hospital costs are poorly covered by the compulsory insurance, you are strongly advised to take out a separate hospital insurance policy. The mutual organisations and private insurers (such as DKV, AG Insurance, Axa and Van Breda) offer hospital packages which allow customers to choose their insurance in function of their needs and financial means.

How much does your mutual insurer reimburse?

When you go to your doctor, you are required to pay the full amount; your mutual insurer then reimburses you a part of the costs.

  • The amount you pay a healthcare provider for an appointment is the honoraire (fee).
  • The amount reimbursed by your mutual insurer for an appointment is l’intervention de l’assurance obligatoire soins de santé (reimbursement by compulsory health insurance).
  • The difference between the official amount of the fee and the amount reimbursed by your mutual insurer is your “quote-part personnelle” (personal share), also called “ticket modérateur”.


Nomenclature of medical services

The nomenclature of medical services (“la nomenclature des prestations de santé”) is the list of health services which are totally or partially reimbursed by your mutual insurer.

Each of these services is defined by a 6-figure code. In addition to the code, the list gives you the complete name of the service and relevant explanations.

After treatment, your doctor, dentist, physiotherapist or other healthcare provider gives you a medical certificate with the code for each treatment.  Hospital bills also include the codes of all treatments provided.

On the INAMI (Belgian National Institute for Health and Disability Insurance) website, a “Nomensoft” database allows you to enter the code mentioned on your medical treatment certificate and obtain the following:

  • A description of the medical service;
  • The fee for that service from a healthcare provider who is “conventionné” (contracted) to the social security system (i.e. one who charges official INAMI rates);
  • The amount of the reimbursement to which you are entitled.

De ziekteverzekering neemt de kosten van een groot aantal zorgprestaties op zich (raadplegingen bij een arts, geneesmiddelen,...). Hoe krijgt u toegang tot dit zorgenstelsel? Hoeveel krijgt u terugbetaald?

Inschrijving bij een ziekenfonds

In België is de inschrijving in de gezondheidszorg niet automatisch, maar vereist ze dat u zich aansluit bij een instelling die « ziekenfonds » wordt genoemd en waaraan u een bijdrage betaalt (+/- 10€/maand).

De voornaamste rol van de ziekenfondsen in België bestaat in een gedeeltelijke terugbetaling van medische en gezondheidskosten en de betaling van een financieel supplement bij arbeidsongeschiktheid (ziekte, moederschapsrust, …). Dit heet de verplichte verzekering. Deze wordt bij wet geregeld en is voor alle ziekenfondsen identiek.

De ziekenfondsen verzorgen eveneens een aanvullende verzekering en bieden meerdere voordelen en diensten bovenop de dienstverlening vervat in de verplichte verzekering. Het gaat daarbij om forfaitaire tussenkomsten voor acupunctuursessies, prothesekosten, oogheelkunde, brilmonturen... Deze aanvullende verzekering verschilt naargelang het ziekenfonds. U kunt zelf uitmaken welke het best tegemoetkomt aan uw behoeften!

Ziekenfondsen zijn verenigingen zonder winstoogmerk die verenigd zijn in landsbonden:

U kunt er ook voor kiezen om aan te sluiten bij een gewestelijke dienst van de Hulpkas voor Ziekte- en Invaliditeitsverzekering (HZIV).  De Hulpkas is een openbare instelling met dezelfde opdrachten als een ziekenfonds die dezelfde prestaties verstrekt (behalve voor de aanvullende verzekering).

OPGELET Aangezien hospitalisatiekosten slechts in beperkte mate gedekt worden door de verplichte verzekering is het sterk aanbevolen om een hospitalisatieverzekering te nemen! Zowel de ziekenfondsen als privéverzekeringen (DKV, AG insurance, Axa, Van Breda, …) bieden hospitalisatiepackages aan zodat de klant zijn verzekering kan kiezen in functie van zijn behoeften en zijn financiële middelen.

Welk bedrag betaalt uw ziekteverzekering u terug?

Wanneer u bij de dokter langsgaat, dient u het volledige bedrag te betalen, waarvan uw ziekenfonds achteraf een deel terugbetaalt.

  • Wat u aan de zorgverlener betaalt voor een prestatie heet het ereloon.
  • Het bedrag waarvoor uw ziekenfonds tussenkomt voor een prestatie is de tegemoetkoming van de verplichte verzekering voor geneeskundige verzorging.
  • Het verschil tussen het officieel ereloontarief en wat uw ziekenfonds u terugbetaalt, is uw persoonlijk aandeel (ook wel remgeld genoemd).


Nomenclatuur van de geneeskundige verstrekkingen

De « nomenclatuur van de geneeskundige verstrekkingen » is de lijst met zorgprestaties waarvoor uw ziekenfonds tussenkomt (volledige of gedeeltelijke terugbetaling).
Aan elke prestatie is een zescijferige nomenclatuurcode verbonden. Naast de code vermeldt de nomenclatuur tevens de volledige benaming van de geneeskundige verstrekking en de toelichting daarbij.

Op het getuigschrift voor verstrekte hulp dat de arts, tandarts, kinesitherapeut enz. u overhandigt, vermeldt deze de code die overeenstemt met elke verstrekte prestatie. Ook uw ziekenhuisfactuur vermeldt de codes van de geneeskundige verstrekkingen die u zijn toegediend.

Op de website van het RIZIV hebt u de beschikking over de gegevensbank « Nomensoft » om de nomenclatuurcode in te voeren die op uw getuigschrift voor verstrekte hulp vermeld staat. U verkrijgt vervolgens:

  • De beschrijving van de geneeskundige verstrekking;
  • Het ereloon dat met deze prestatie overeenstemt bij een geconventioneerde zorgverlener (d.w.z. een arts die de officiële, door het RIZIV vastgestelde tarieven aanvaardt en toepast);
  • Het bedrag van de terugbetaling waarop u recht hebt.
Dit kan dus een nuttig instrument zijn!

S’inscrire auprès d’une mutualité

En Belgique l’affiliation au système de soins de santé n’est pas automatique mais nécessite une inscription auprès d’un organisme appelé « mutualité » ainsi que le paiement d’une cotisation (+/- 10€/mois).

Le principal rôle des mutualités en Belgique est d’assurer un remboursement partiel des frais médicaux et de santé, et de donner un complément financier en cas d’incapacité de travail (maladie, repos de maternité, …). Cela s’appelle l'assurance obligatoire. Elle est régie par la loi et est identique pour toutes les mutualités.

Les mutualités proposent également une assurance complémentaire et offrent ainsi divers avantages et services en plus de ceux prévus par l’assurance obligatoire. Il s’agit d’interventions forfaitaires pour des séances d’acuponcture, des frais de prothèses, chirurgie oculaire, montures de lunettes... Cette assurance complémentaire varie d’une mutualité à l’autre. A vous de comparer laquelle est la plus intéressante en fonction de vos besoins !

Les mutualités sont des associations sans but lucratif, qui se regroupent en unions nationales :

Vous pouvez également choisir de vous inscrire auprès d’un office régional de la Caisse auxiliaire d'assurance maladie invalidité (CAAMI).  Cette Caisse auxiliaire est une institution publique qui remplit les mêmes missions qu’une mutualité et qui octroie les mêmes prestations (sauf pour l’assurance complémentaire).

ATTENTION les frais d’hospitalisation étant très peu couverts par l’assurance obligatoire il est fortement conseillé de souscrire une assurance hospitalisation ! Les mutuelles ainsi que des assurances privées (DKV, AG insurance, Axa, Van Breda, …) offrent des packages hospitalisation qui permettent au client de choisir son assurance en fonction de ses besoins et de ses moyens financiers.

Quel montant vous rembourse votre mutualité?

Lorsque vous vous rendez chez votre médecin, vous devrez payer la totalité du montant et ensuite votre mutualité vous remboursera une partie des frais que vous avez payés.

  • Ce que vous payez à votre dispensateur de soins pour une prestation est l’honoraire.
  • Le montant de l’intervention de votre mutualité pour une prestation est l’intervention de l’assurance obligatoire soins de santé.
  • La différence entre le tarif officiel de l’honoraire et ce que votre mutualité vous rembourse est votre quote-part personnelle (appelée aussi ticket modérateur).


Nomenclature des prestations de santé

La « nomenclature des prestations de santé » est la liste des prestations de santé pour lesquelles votre mutualité intervient (remboursement total ou partiel).
Un code de nomenclature, composé de 6 chiffres, désigne chaque prestation. À côté du code, la nomenclature vous donne l’intitulé complet de la prestation et les explications qui la concernent.

 Sur l’attestation de soins qu’il vous remet, votre médecin, dentiste, kinésithérapeute, etc. écrit le code correspondant à chacune des prestations qu’il vous a données. Votre facture d’hospitalisation reprend également les codes de toutes les prestations reçues.

Sur le site de l’INAMI une base de données « Nomensoft » vous permet d’introduire un code de nomenclature repris sur votre attestation de soins et d’obtenir :

  • la description de la prestation de santé
  • l’honoraire pour cette prestation chez un dispensateur de soins conventionné (à savoir un médecin qui accepte et applique les tarifs officiels établis par l’INAMI)
  • le montant du remboursement auquel vous avez droit.
